After being accepted into the tournament, you will be required to submit various required paperwork. All required paperwork will be submitted on GotSoccer.
Team Check-In is an easy process. Just log onto GotSoccer with your team account, navigate to the Discovery Cup 2017 tournament page within your team account, and click on documents. There you will be able to upload your documents.
Each team must submit a roster. All teams must have an official club/state roster from USYSA or US Club. You may write-in the name of any guest players on the bottom of your club/state roster (please note only 5 guest players are allowed per team).
Waivers - you have TWO options, please only choose ONE:
- Waiver (one much be filled out for EACH player)
- Group Waiver form - one form PER team - all lines MUST be filled out
Please note Medical Release Forms ARE NOT the same as Waivers. The waivers are liability waivers for the Maryland SoccerPlex ONLY - one individual waiver must be filled out for each player or one group waiver must be filled out her team; NO WAIVERS=NO PLAY!
Every participating player must also submit a player card - either from USYS or US Club. PLEASE NOTE teams may not have rosters that are comprised of players from USYS and US Club - no mixed rosters will be accepted.

All players must have a medical release form on file with their coach/team manager for the duration of the tournament; however, these medical forms DO NOT need to be submitted during registration. If you DO NOT have a medical release form please fill one out and keep it with you for the duration of the tournament. The form is listed above.
PLEASE UNDERSTAND THAT EACH OF THESE DOCUMENTS WILL BE CHECKED AT REGISTRATION. YOUR FAILURE TO PROVIDE ANY OF THESE NECESSARY DOCUMENTS WILL PRECLUDE YOUR TEAM’S PARTICIPATION IN THE TOURNAMENT.
The Tournament Committee will retain a copy of your certified team roster.
Players not attending the event:
If a player is not attending the event do I leave their name on the roster?
No, if a player is not attending the event, you need to cross their name off of the roster, preferably in thick, black permanent marker.
What is the maximum number of guest players a team can bring to the event?
No team may have more than 5 guest (or loan) players.
For US Club Teams ONLY:
- A copy of each US Club Player Pass for guest (or loan) players (front & back must be copied)
For US Youth Soccer Team ONLY:
- A copy of each USYS Player Pass for guest (or loan) players
All other relevant registration materials listed above must also be provided for EACH guest (or loan) player.
The following are the maximum roster size per age groups:
U09 – U10 (7v7) – 14 players
U11-U12 (9v9) – 16 players
U13 (11v11) – 18 players
U14 - U19 (11v11) - 22 Players, but only 18 may dress for any given game.
How do I add a guest player?
Step 1: Obtain the guest player's current player card and medical release form
Step 2: Photocopy your original roster. Please hand write the guest player’s full name, unique jersey number, player identification number and birthday below the bottom of the existing rostered players.
Step 3: Fill out the Loan Player form listed above
Note: US Club Soccer teams may only take guest players registered under US Club Soccer, and USYS teams may only take guest players registered with properly stamped USYS player pass cards; recreation players are not permitted. No roster may be comprised of players with different passes from different organizations (no “mixed rosters”).
Medical Release Forms
Do I include the medical release forms in the on-site registration packet?
All teams must have a medical release form for each player (including guest players) but DO NOT include the medical release forms in the registration packet.
If you DO NOT have a medical release form please fill one out and keep it with you for the duration of the tournament. The form is listed above.
