Notes:

• The entry fee is required of each team at the time of application
• Applications submitted without payment will not be considered for entry
• Applications and fees must be post-marked by August 15, 2010
• Refund Policy: Teams withdrawing from the tournament after acceptance will not be refunded the entry fee.
