Teams (U15-U19 age groups) wishing to be included in the College Profile Book must use the attached form to be submitted NO LATER than September 22nd. Please complete the form as an Excel Document, save it to your computer, email form to director@discoverycup.com. PLEASE DO NOT MODIFY THE FORM.
